ADMISSION REQUIREMENTS
Seven- Month Dental Externship Program in Oral Surgery and Hospital Dentistry
Applicants desiring admission to the program must submit the following requirements for review by the Admission Committee
1) Completed application form
2) Two (2) 2'x2" pictures
3) Certified true copy of the applicant's Transcript of Records
4) Certified true copy of diploma
5) Certified true copy of dental board rating
6) Certified true copy of Certificate of Registration from the PRC
7) Two (2) sealed letters of recommendation addressed to the Chairman of the Department of Dentistry. One letter must come from the Dean of the Dental School from which the applicant graduated and one from a dental faculty attesting that the applicant has the ability and good moral character to pursue the program.

8) Passing score in the Evaluation Examination with a grade of at least 75%
9) A short essay of not more than 200 words and not less than 150 words as to why the applicant is seeking admission to the program
10) PHP500 application fee
11) Submit to a personal interview scheduled by the Admission Committee
NOTE: ll admission requirements must be submitted not later than one (1) month before the starting date of
a) the program period. There are 4 periods of admission per year:
First: January batch
Second: April batch - Examination on March 30, 2022
Third: July batch - Applications in June
Fourth: October batch - Applications in September
b) Admission examinations are given four times a year.
c) Application requirements which are incomplete or incorrect and falsified will forfeit applicant for admission.
d) Kindly submit all the requirements in a folder with plastic jacket.
